We collect a variety of personal information to help us in administering the highest quality of healthcare. This information is provided by members in our triage forms and surveys (such as address, social security number and dependant information). We retain this information after a patient is seen. We limit the collection of personal information to that which is necessary to administer care, provide quality service and meet regulatory requirements.

**In Short:** Only as long as necessary for the purposes in this notice (or as required by law). When no longer needed, we delete or anonymize it.

**In Short:** We use reasonable organizational and technical safeguards, but no method is 100% secure. Transmission is at your own risk.

**No.** We do not knowingly collect data from anyone under 18. If we discover we have, we will delete it immediately.

How We Protect Personal Information
We treat personal information securely and confidentially. We limit access to personal information to only those persons who need to know that information to provide our products or services to members (for example, our referral services and care coordinators). These persons are trained on the importance of safeguarding this information and must comply with our procedures and applicable law. We meet strict physical, electronic, and procedural security standards to protect personal information and maintain internal procedures to promote the integrity and accuracy of that information.

We may share any of the personal information we collect (as described above) with our affiliates as permitted by law. We have procedures for a patient to access the personal information we collect, and we will make this information available to the patient upon request.
Our goal is to keep our patient information up-to-date and to correct inaccurate information.
